Assessments
INITIAL NEEDS
Total Access is able to provide comprehensive Initial Needs Assessments (INA) and reports to assist a client with a smooth transition between hospital and home. Each assessment is conducted in the comfort of the client’s home (and workplace if required) however, depending on the needs of the client, we are able to initiate our assessments whilst the client is still in hospital.
The INA addresses Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) tasks, providing short term, client-centred goals. This assists the client in remaining focused on their long term goals of functional independence.
PRESCRIPTIVE ASSESSMENTS
Once the client has returned home, we offer further assessments to ensure maximal functional independence is maintained. Assessments are offered in the following areas:
- Wheelchair and equipment prescriptions
- Seating and pressure care
- Prescription of aids and equipment
- Home environment and access
- Activities of Daily Living (ADL) review
- Instrumental Activities of Daily Living (IADL) review
